The Registered Nurse assesses patient needs, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates care delivery throughout the patient’s episode. The Registered Nurse develops specialized skills in the area of patients admitted for medical conditions. He / She provides care to adult, adolescent, and geriatric populations with medical conditions. The Registered Nurse is accountable for patient and family education to assist in understanding self-care beyond hospitalization, including lifestyle changes to support health and healing. The Registered Nurse is expected to promote comfort and provide emotional support in a healing environment. The nurse applies nursing knowledge and skill in the delivery of care. The nurse assesses patient and family needs through active listening, social perceptiveness, and analyzing data or information obtained through direct observation, assessment, technology, or communication with the health team. The nurse creates a developmentally and age-appropriate, culturally relevant, individualized plan of care to address the nature and scope of the health problems identified. In collaboration with the patient, family, physician, and healthcare team members, the nurse sets goals and monitors the patient’s progress. The nurse adjusts the plan based on patient response or change in condition. The nurse promotes, advocates for, and protects the health, safety, and rights of the patient. The nurse encourages and builds mutual trust, respect, and cooperation among team members to accomplish the best possible patient outcomes. The nurse teaches others as appropriate and delegates tasks and interventions consistent with the individual’s education, training, and scope of practice. The nurse understands personal responsibility to safeguard public safety through compliance with infection prevention strategies. The nurse motivates the patient and family by teaching them what is needed to continue care and healing at home. The nurse assesses the patient and family learning styles and structures the sessions to maximize understanding. The nurse seeks teachable moments during the care delivery process to emphasize pertinent information. The nurse asks questions to evaluate retention of key points in the self-care process. The nurse provides tools and information to assist the patient’s family after discharge. The nurse documents the education provided and the patient and family response to the education. The nurse systematically evaluates the quality and effectiveness of nursing practice and uses the results of quality improvement activities to initiate changes in individual nursing practice and care delivery. The nurse engages in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial. The nurse takes accountability to increase knowledge and enhance skill. The nurse uses logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
