Salary : Position Summary
The Licensed Practical Nurse (LPN) provides clinical oversight and direct nursing support for Host Home and PCA clients. Responsibilities include conducting nurse delegations in accordance with theColorado Nurse Practice Act, medication monitoring and setup, training non-licensed caregivers, and ensuring high-quality care for members with intellectual and developmental disabilities.
Key Responsibilities
Clinical Oversight & Nurse Delegation
- Complete assessments and determine appropriate nurse delegations per theColorado Nurse Practice Act
- Delegate and train caregivers on health-related tasks (e.g., medication administration, G-tube care, trach suctioning, oxygen use, injections)
- Maintain records of all delegations and ensure timely re-assessments
- Ensure delegated caregivers are operating within their scope and competence
Medication Oversight
Conduct routinemedication monitoringto ensure correct administration and documentationPerformmed countsand verify accuracy of medication administration records (MARs)Set up medicationsin pill planners or medication boxes, as permitted under nurse delegation rulesEnsure medication storage and handling meet regulatory standardsCommunicate with providers and pharmacies regarding changes in medication ordersDocumentation & Monitoring
Complete visit notes, health summaries, and monitoring forms for each clientTrack expiration of delegations, training, and other compliance documentsIdentify and report health-related issues or medication discrepanciesMaintain thorough and timely documentation in electronic systems (e.g., Therap)Training & Compliance
Provide training to caregivers on standard precautions, infection control, and emergency responseEducate providers on recognizing signs of health decline and when to report changesAssist in developing corrective action plans when non-compliance is identifiedSupport audit readiness by ensuring clinical documentation is complete and currentCollaboration & Care Coordination
Communicate regularly with interdisciplinary teams, guardians, case managers, and physiciansParticipate in service planning meetings and help advocate for appropriate medical supportsRespond to urgent medical issues or incidents and provide clinical guidance when neededRequired Qualifications
Active, unrestricted Colorado LPN licenseAt least 1 year of LPN experiencein home health, IDD, or community-based settingsKnowledge of theColorado Nurse Practice Actand applicable nurse delegation protocolsValid drivers license, reliable transportation, and proof of insuranceAbility to pass all required background and registry checksStrong organization, communication, and independent time-management skillsProficiency in electronic documentation (Therap experience preferred)Preferred Qualifications
Prior experience with medication setup and med counts in residential or IDD settingsFamiliarity with DD, SLS, or CES waivers in ColoradoSkilled in G-tube, trach, oxygen management, or insulin administrationBilingual (Spanish / English) a plusCompensation & Benefits
