Key Responsibilities as a Home Health RN :

• Deliver skilled nursing care to patients in their homes

Conduct initial assessments and tailor care plans to individual needs

Administer medications, IV therapy, wound care, and other essential treatments

Provide education to patients and caregivers on chronic condition management, medication use, diet, and post-acute care

Coordinate care with healthcare providers, case managers, and the home health team

Maintain timely and accurate clinical documentation

What You Need to Qualify :

Minimum two years of nursing experience

Preferred home health experience (minimum one year ideal)

Reliable transportation and a valid driver’s license

Compassion, clinical proficiency, and excellent communication skills
