About the Role

As the RN Care Manager, you are a part of a revolutionary service coordinating all patient care between primary, specialty, inpatient and behavioral health services. You serve a variety of patients including individuals with substance use disorders and both children and adults with mental illness.

Your role is to assist clients in initial and ongoing care coordination including :

CWN prioritizes work and life balance. Your supervisor will work with you to utilize flex time and paid time off. You will primarily work in the office with the following schedule : Monday through Friday Between 8 : 00am and 5 : 00pm

You may be required to work hours outside of the regular schedule if assigned by your supervisor to meet clients’ needs.
