Job Details

The RN Case Manager is responsible for coordinating and delivering skilled nursing care and oversight for patients receiving home health services. You’ll manage a caseload of patients, perform comprehensive OASIS assessments, and develop individualized care plans in collaboration with physicians, therapists, and home health aides.

Key Responsibilities :

Conduct initial and ongoing OASIS assessments

Develop, implement, and evaluate individualized care plans

Communicate regularly with physicians and multidisciplinary team members

Educate patients and caregivers on disease management and home safety

Ensure compliance with CHHA and regulatory standards
