Job Description

Company Overview :

Public Health Solutions (PHS) is a 501(c)3 non-profit community-based organization (CBO) that has existed for 70 years to improve health equity and address health-related social needs (HRSN) for historically underserved marginalized communities. As the largest public health nonprofit serving New York City, we improve health outcomes and help communities thrive by providing services directly to vulnerable families, supporting community-based organizations through our long-standing public-private partnerships, and bridging the gap between healthcare and community services. We focus on a wide range of public health issues including food and nutrition, health insurance, maternal and child health, sexual and reproductive health, tobacco control, and HIV / AIDS. Learn more about our work at healthsolutions.org.

PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.

New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.

This is a grant-funded position ending March 31, 2027.

Program Description :

The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS’ experience in sexual and reproductive health and healthcare-community partnerships to the execution of scalable healthcare capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The HCP Capacity Building team will support the trustworthy integration of health-related social needs (HRSN) screening and referral to community resources into a broad range of healthcare practice settings within PHS’ SCN, including hospital care, primary care, sexual and reproductive healthcare, behavioral health, and care management programs.

Position Summary :

We seek an experienced and systems-oriented Practice Improvement (PI) Specialist to join our HCP Capacity Building team, with a focus on supporting healthcare provider interoperability within PHS’ SCN. This individual will serve as the primary interoperability expert and will lead efforts to integrate HRSN screening into provider systems through data exchange with the SCN IT platform. The PI Specialist will provide targeted Quality Improvement (QI) coaching, technical assistance (TA), and implementation support to healthcare partners. The ideal candidate will bring deep experience with healthcare data systems, an understanding of public health and health equity frameworks, and a demonstrated ability to engage and align diverse partners across clinical, community, and state systems.

The PI Specialist will report to the QI and Evaluation Manager and will work in close collaboration with the team’s other PI Specialists and the rest of the HCP Capacity Building team.

Specifically, the PI Specialist will :

  • Serve as the interoperability lead within the HCP-CB team, responsible for guiding the implementation and optimization of HRSN-related data exchange workflows across healthcare partners.
  • Work in partnership with other PI Specialists to provide TA and improvement coaching to healthcare providers, with an emphasis on aligning interoperability approaches with SCN IT platform capabilities and state-defined data standards.
  • Represent the HCP-CB team in cross-sector meetings, including regular collaboration with :

  • Regional Qualified Entities (e.g., Healthix, Bronx RHIO, Hixny, etc.), to coordinate provider-level Health Information Exchange (HIE) onboarding and integration
  • The Social Care Network IT platform vendor (Unite Us), to troubleshoot data flows, develop use cases, and align provider-facing processes with back-end system capabilities
  • Analyze and interpret interoperability-related performance data (e.g., referral completion, HRSN data capture rates) in collaboration with PHS’ Quality & Evaluation and Care Coordination teams and use insights to drive iterative improvements.
  • Support the development of QI materials and tools to help providers assess and build interoperability capacity.
  • Co-develop and facilitate learning sessions and training sessions focused on technical readiness, workflow redesign, and best practices for HRSN screening and referral integration.
  • Contribute to the design of the Quality Improvement Learning Collaborative and broader QI strategy discussions, including ways to support participants to share successful improvement practices and facilitate collaborative learning.
  • Build and maintain strong relationships with healthcare partner improvement teams.
  • Prepare reports, presentations, and other documentation, as required.
  • Qualifications and Experience :

  • Bachelor’s degree required; Master’s in Public Health, Health Informatics, or a related field preferred.
  • 2-3 years of experience in QI, capacity building, technical assistance, or systems implementation – ideally supporting healthcare data system or interoperability efforts.
  • Familiarity with New York’s health information exchange environment, with a preference for experience working with NYeC or Regional Qualified Entities.
  • Experience engaging with external stakeholders such as hospitals, Federally Qualified Health Centers, behavioral health providers, and / or community-based organizations.
  • Understanding of platforms and standards such as FHIR, HL7, Direct Secure Messaging, SMART on FHIR, or HIE integration pathways.
  • Experience supporting providers in implementing social care-related screenings or value-based care initiatives preferred.
  • Desired Skills :

  • Strong knowledge of QI methodologies, capacity building strategies, and public health principles.
  • Strong understanding of interoperability and data governance frameworks, particularly as applied to social care integration.
  • Excellent verbal and written communication skills, with the ability to convey complex information clearly and effectively. Demonstrated ability to engage technical and non-technical audiences alike.
  • Strong stakeholder engagement and relationship-building skills across healthcare and technology partners. Ability to work independently and as part of a team, managing multiple priorities and deadlines.
  • Highly organized and detail-oriented with exceptional time and project management skills.
  • Proficiency in Microsoft Word, Excel, PowerPoint and use of web-based meeting applications.
  • Commitment to improving health equity, access and quality of care within healthcare practice settings in NYC .
  • Benefits :

  • Hybrid Work Schedule.
  • Generous Paid Time Off and Holidays.
  • An attractive and comprehensive benefits package including Medical, Dental and Vision.
  • Flexible Spending Accounts and Commuter Benefits.
  • Company Paid Life Insurance and Disability Coverage.
  • 403(b) + employer matching and discretionary company contributions.
  • College Savings Plan.
  • Ongoing trainings and continuous opportunities for professional growth and development.
  • Salary $72,000 to $72,000
  • At PHS, we place immense value on diversity within our teams, understanding that varied backgrounds and experiences significantly enhance our community and propel us toward our goals. If you find you don’t have experience in all the areas listed above, we still encourage you to apply and share your background and experiences in your application.
