PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.
This is a grant-funded position ending March 31, 2027.
Program Description :
The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS’ experience in sexual and reproductive health and healthcare-community partnerships to the execution of scalable healthcare capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The HCP Capacity Building team will support the trustworthy integration of health-related social needs (HRSN) screening and referral to community resources into a broad range of healthcare practice settings within PHS’ SCN, including hospital care, primary care, sexual and reproductive healthcare, behavioral health, and care management programs.
Position Summary :
We seek an experienced and systems-oriented Practice Improvement (PI) Specialist to join our HCP Capacity Building team, with a focus on supporting healthcare provider interoperability within PHS’ SCN. This individual will serve as the primary interoperability expert and will lead efforts to integrate HRSN screening into provider systems through data exchange with the SCN IT platform. The PI Specialist will provide targeted Quality Improvement (QI) coaching, technical assistance (TA), and implementation support to healthcare partners. The ideal candidate will bring deep experience with healthcare data systems, an understanding of public health and health equity frameworks, and a demonstrated ability to engage and align diverse partners across clinical, community, and state systems.
The PI Specialist will report to the QI and Evaluation Manager and will work in close collaboration with the team’s other PI Specialists and the rest of the HCP Capacity Building team.
