Reentry Assessment Coordinator

Description

The Reentry Assessment Coordinator is responsible for assessing justice-involved individuals for placement within the STOP (Specialized Treatment for Optimized Programming) Services program based on identified criminogenic risks and needs, including substance use disorders (SUD) and related behavioral health factors.

This position works directly out of designated parole offices within the assigned program area and serves as a liaison between parole, treatment providers, and STOP program staff to ensure appropriate, timely, and effective program placement. The Reentry Assessment Coordinator plays a critical role in matching participants with the most suitable treatment modalities to support rehabilitation, stabilization, and reduced recidivism. Essential job functions are listed below.

Essential Job Functions:

  • Conduct criminogenic risk and needs assessments to identify dynamic risk factors and service needs, with an emphasis on substance use disorders and related behavioral health concerns.
  • Utilize validated assessment tools and evidence-based screening instruments in accordance with program and state guidelines.
  • Evaluate participants' readiness for treatment, responsivity factors, and barriers to engagement.
  • Document assessment findings clearly, accurately, and in compliance with program and regulatory standards.
  • Generate individualized placement recommendations based on assessment results, aligning participants with appropriate STOP program modalities (e.g., outpatient treatment, residential services, cognitive-behavioral programming, case management).
  • Ensure recommendations are consistent with risk-needs-responsivity (RNR) principles and evidence-based practices.
  • Collaborate with parole officers and program staff to coordinate referrals and ensure timely, appropriate placements.
  • Assist with placement logistics and support, verifying program capacity and participant readiness based on assessment outcomes.
  • Monitor referral progress and address barriers to successful program entry.
  • Work onsite within designated parole offices to facilitate communication and partnership.
  • Participate in multidisciplinary case conferences and reentry planning meetings.
  • Serve as a liaison between parole, STOP program staff, treatment providers, and other community partners.
  • Provide education to stakeholders regarding STOP program services, eligibility criteria, and referral processes.
  • Must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
  • Perform all other duties as assigned.

Qualifications

Essential Qualifications:

To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Experience and Competencies:

  • Knowledge of criminogenic risk factors, substance use disorders, and reentry challenges.
  • Familiarity with California parole systems and community-based treatment resources preferred.
  • Strong documentation and organizational skills.
  • Ability to work independently within a parole office setting.
  • Professional appearance and demeanor.
  • Knowledge of evidence-based treatment modalities for justice-involved populations.
  • Strong assessment, analytical, and critical thinking skills.
  • Ability to maintain professional boundaries and work effectively in correctional settings.
  • Excellent interpersonal and collaboration skills.
  • Strong written and verbal communication abilities.
  • Ability to obtain and maintain clearance from California Department of Corrections and Rehabilitation (CDCR).
  • Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required.
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.
  • During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

Education and/or Experience:

  • Minimum two (2) years' experience conducting assessments with justice-involved populations, ideally with a focus on criminogenic needs and/or substance abuse.
  • Highschool diploma or its' equivalent is required.

Licensure/Certification:

  • Not applicable for this position.

Working Conditions:

  • Primary work location within designated parole offices.
  • Frequent interaction with justice-involved individuals.
  • May require local travel to treatment providers or partner agencies.
  • Standard office environment with occasional exposure to correctional settings.

Essential Physical and Mental Demands of the Job:

The employee must be able to perform the following essential duties and activities with or without accommodation:

Physical Demands:

  • Requires mobility and physical activity: Having an adequate range of body motion and mobility to work in an office, residential, or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended periods of time, bending, twisting, reaching, balancing, occasional lifting and carrying of up to 25 pounds. Use of computer and telephone systems is required which includes coordination of eye and hand, and fine manipulation by the hands (typing, writing, and working with files). Requires the ability to defend oneself and clients in physically abusive situations using approved physical de-escalation techniques.
  • Requires talking: Expressing or exchanging ideas by means of the spoken word. Talking is required to impart oral information to employees, clients, patients, and the public, and in those activities in which the employee is required to convey detailed or important spoken instructions to others accurately, loudly, or quickly.
  • Requires hearing: Hearing is required to receive and communicate detailed information through oral communication.
  • Requires seeing: Clarity of vision at 20 inches or less and at distance. This factor is required to complete paperwork for many of the employee's essential job functions and to observe client behavior and activities in and out of the facility.
  • The normal work routine involves no exposure to human blood, body fluids or tissues. However, exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee.

  • Requires the ability to collect and analyze complex numerical and written data and verbal information to reach logical conclusions.
  • Requires the ability to work and cooperate with clients, co-workers, managers, the public and employees at all levels to exchange ideas, information, instructions, and opinions.
