Case Manager RN; Ratio 1 : 25-30 Adults & Geriatrics; 5x8hr Days; 830a-5p Mon-Fri plus one full wknd every 4 weeks; No Call or Holidays

Two years recent acute care hospital CM experience and Discharge Planning required; Level 1 Trauma experience preferred; Must be proficient in Microsoft Office Suite, CarePort and AllScripts and Powerchart. Will be required to complete initial assessment and then revisit pts minimum of every third day.
