Job Description

This position typically reports to a Program Director, Assistant Program Director, or other appropriate project administrator within the unit. A Community Outreach Caseworker may be assigned duties with a different focus than another Caseworker, but the duties are interrelated and contribute to the purpose / goals of the program. One caseworker may concentrate on educating pregnant or postpartum women on infant mortality and morbidity, while another caseworker my focus on addressing the psychosocial problems of perinatal depression in women. This position will go into the community - into client homes, to community gathering places, churches, and other local spots to locate and interview the clients or potential clients who most need their services. Although each position is responsible for its assigned cases, the Community Outreach Caseworker also serves as a team member to develop multidisciplinary solutions to client problems. This position is non-exempt. While this position works with substantial independence in the field, it is limited to routine decision-making with regard to client services, operating within established policy guidance. Unusual situations are discussed with supervisor prior to taking action. This position is time-limited. This position requires a level II background check.

Responsibilities

  • Identifies eligible participants for the program, interprets standards and guidelines to potential clients, answers questions, obtains consent for services, and makes referrals based upon program requirements.
  • Interviews clients and their families to collect data, disseminate health education information, and administer satisfaction surveys and related evaluative inventories.
  • Determines need and makes recommendations for continuation of or change in services.
  • Provides case management for clients who have been identified as having mainly psychosocial problems; refers and follows up on the receipt of services; coordinates, consults and collaborates with service providers.
  • Administers depression screens to clients; provides in-depth interconception health education, ongoing support and guidance in child health development to clients, partners / spouses and families.
  • Participates in mentoring training; may develop and implement mentoring activities for other caseworkers.
  • Works with clients and families to determine need for and encourage participation in psychoeducational classes, health-related classes, and support groups. Facilitates non-clinical group discussions as needed, and provides support for groups and health-related classes for clients and alumni.
  • Arranges for transportation for and encourages attendance at health care appointments, health education and psychoeducational classes, and social service related appointments.
  • Documents records regarding case management / counseling activities and termination as appropriate; prepares required reports - caseload reports, case logs, etc.
  • Establishes contact with and serves on committees of agencies and organizations serving pregnant and postpartum women and infants in project area.
  • Participates in monthly Community Council meetings, participates as a member of multi-disciplinary Care Coordination team meetings to offer total client care, attends staff meetings. Makes reports of all activities.
  • Travels as required for home visits and other community activities.

Qualifications

This position requires a high school diploma or equivalent, with three years experience in office, administrative, or customer services positions. Appropriate college coursework or vocational / technical training may substitute at an equivalent rate for the required experience.
