Responsibilities

  • Reach out to patients following a recent hospital visit and connect them with a registered nurse
  • Obtain and document basic information about a patient's recent hospital stay
  • Review and prioritize patient census for effective outreach
  • Explain services and enroll patients into appropriate programs based on their needs
  • Address inquiries and schedule initial and annual appointments for patients

    • Qualifications

  • High School diploma or equivalent
  • Minimum of 1 year of experience in a call center, sales, healthcare, or customer-focused environment
  • Ability to work in a virtual setting with flexibility regarding time zones (MST and CST preferred)
  • Strong communication skills to effectively engage with patients
  • Proficient in entering documentation into designated systems

    • Preferred Qualifications

  • Bilingual in English and any of the following languages : Spanish, Mandarin, Cantonese, Russian, Korean, or Gujrati / Hindi
