Georgia Licensed Care Coordination Nurse

A company is looking for a Care Coordination Nurse I- AWV (LPN).

Key Responsibilities

Manage inbound and outbound patient interactions to complete Medicare Annual Wellness Visit forms

Utilize customer systems to identify upcoming AWV appointments and assist with patient education

Ensure compliance with privacy standards and complete necessary documentation for services

Required Qualifications

Licensed Practical Nurse (LPN) license required, preferably in the State of Georgia or compact multi-state license

Minimum 3 years of experience in a doctor's office or hospital setting

High school diploma required; college or some college preferred

Experience in the healthcare field with knowledge of medical terminology

Typing speed of at least 55 wpm; multilingual English / Spanish preferred
