Community Health Worker II (Non-Exempt)

This trusting relationship enables the worker to serve as a liaison / link between health / social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Qualifications :

• Experience : 1-3 years CHW experience
• Required Education : High School Diploma or equivalent
• Certifications : Basic Life Support
• Other : Strong relationship and collaboration skills, Excellent communication skills, Ability to work independently, Compassion for poor and underserved members of the community, Epic electronic medical record
