DETAILS :

Role will be at our newly built outpatient facility starting in Sept 2025

Connected to our independent living facility

Patient-focused, relationship-driven care model

Support staff in place to help you with all other duties

Weekly physician visits and easy access to physicians via phone

Open to full or part-time providers

Experience working with geriatric patients as an NP for 3+ years required
