Internal Medicine Physician

AbsoluteCare is seeking a compassionate and mission-driven Primary Care Physician to join our Chicago team. In this role, you ll provide comprehensive, value-based care to a panel of medically and socially complex patients seeing just patients per day to allow for deeper relationships and better outcomes.

Our model is built around the Patient-Centered Medical Home (PCMH) and integrates medical, behavioral, and social services under one roof. We care for patients who are often overlooked by traditional systems those with chronic conditions, behavioral health needs, and social challenges like housing instability and food insecurity.
