Job descriptionPerforms ongoing coordination of services to children and adults in accordance with the treatment plan. Responds to initial crisis calls and identifies needs & necessary resources accordingly. Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services while demonstrating a positive, strengths-based attitude of teamwork and shared leadership. Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation / Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers. Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered Relevant experience working in a behavioral-health setting Level 1 Fingerprint Clearance Card Verification of education, employment, references MVR matching JFCS driving guidelines AZ Driver's License and Valid Auto Insurance Generous Employee Referral Bonuses for both the new-hire and the referring employee.
