Participate in interdisciplinary case conferences, discharge planning, and the coordination of care across the rehab team. Implement a plan of care for patients and evaluate the effectiveness of it in an ongoing, systematic process. Supervise physical therapist assistants, rehab therapy aides, and student clinical experiences to ensure quality care and professional development. Guide therapy and intervention through evidence-based research Bachelor's Degree in Physical Therapy required 2 years of experience required PT - State of California Physical Therapist License required BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required Pay ranges listed are an estimate and subject to change. If any bonuses are noted, they are only applicable to external hires meeting criteria.

Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for patients with chronic and subacute conditions — such as coma, ventilator dependence, or complex illnesses — that require 24-hour medical supervision. With a focus on comfort, recovery and rehabilitation, CSTCC blends the effectiveness of acute treatment with the supportive environment of long-term care, helping patients progress toward improved function and quality of life.
As part of the Valley’s largest healthcare system, this role gives you the opportunity to apply advanced therapy skills in a subacute setting while helping patients achieve meaningful recovery goals.
