Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for patients with chronic and subacute conditions — such as coma, ventilator dependence, or complex illnesses — that require 24-hour medical supervision. With a focus on comfort, recovery and rehabilitation, CSTCC blends the effectiveness of acute treatment with the supportive environment of long-term care, helping patients progress toward improved function and quality of life.

As part of the Valley’s largest healthcare system, this role gives you the opportunity to apply advanced therapy skills in a subacute setting while helping patients achieve meaningful recovery goals.
