Care Consultant Sr (IN, KY, OH)
Hybrid: This role requires associates to be in-office 1-2 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace.

The Care Consultant Sr (IN, KY, OH) is responsible for fostering long-term partnerships with our providers and driving enhanced patient outcomes, driving the achievement of value-based care in the Medicare, Medicaid, and Commercial lines of business.
This individual will be consulting with providers, conducting assessments, and deploying interventions to enhance capabilities and performance. This role involves a variety of tasks such as strategy development, implementation support, training, and data analysis to ensure the success of our provider partners.
Estimated travel: Up to 20%; none overnight
