Job description
Providing telephonic coaching and educational resources, the full-time salaried Disease Management Care Coach - Cardiology will support individuals with chronic health conditions by assessing needs, coordinating care, and promoting wellness in a 100% remote environment. Key responsibilities Deliver telephonic coaching and referral services to participants managing chronic health conditions with clinical oversight Assess participant needs and communicate with healthcare professionals for effective care coordination Document coaching sessions and participant activities in the case tracking software Required qualifications Nursing Degree from an accredited college or university Active and unrestricted Compact Registered Nurse License 2+ years of experience in direct participant coaching Intermediate knowledge of Microsoft Office, including Word, Excel, and PowerPoint Experience with information database systems for reviewing medical history and data
