About the Role
We are seeking a Physical Therapist (PT) Home Health for a 13-week contract in Albuquerque, New Mexico. The PT will provide skilled physical therapy services in patients' homes, including evaluations, individualized treatment plans, reassessments, recertifications, and patient and caregiver education.
Home health visits may require travel within approximately a 45-mile radius of the Albuquerque area, including surrounding communities. Reliable transportation is required, and mileage reimbursement is not provided. Home health experience is preferred but not required.
Responsibilities
Perform initial evaluations, progress assessments, 30-day reassessments, and 60-day recertifications in accordance with applicable standards.
Develop, implement, and update individualized physical therapy plans of care based on physician orders and patient needs.
Provide skilled therapeutic interventions and educate patients and caregivers on mobility, home exercise programs, safety, and adaptive strategies.
Complete OASIS documentation, recertifications, transfers, resumptions of care, discharges, and appropriate ICD-10 coding.
Coordinate care with physicians and interdisciplinary team members and supervise Physical Therapist Assistants (PTAs) and Home Health Aides as permitted by state regulations.
Requirements
Current, Active New Mexico Physical Therapist License Required.
Graduate of an Accredited Physical Therapist Program Required.
Current CPR Certification Required.
2 Years of Clinical Experience Preferred; Home Health Experience Preferred but Not Required.
Valid Driver's License, Auto Insurance, and Reliable Transportation Required for Home Health Visits.
