PROGRAM SUMMARY

As the front door to the Hamilton County public behavioral health system Central Connection provides standaridized entry into a system of care that ensures that services are available accessible and high quality. Central Connection staff are available 24 / 7 365 to answer calls and connect consumers to appropriate services. Our primary function is to access authorize and connect individuals and families to the appropriate behavioral health services. Clinicians can provide assessments transitional case management and transitional medication services coordinate referrals provide housing assessments and application assistance for Medicaid and Social Security enrollment.
