Job description
A company is looking for an OB Care Manager (RN) to develop and facilitate complex care management activities for members with physical health needs.
Key Responsibilities
Evaluate member needs and barriers to care, recommending plans for optimal outcomes
Develop and collaborate on ongoing care plans with providers and community resources
Monitor member progress and adjust care plans as necessary to meet goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience in OB, NICU, Postpartum, and / or Nursery
Current RN licensure in Florida or compact state
Experience in care management and member education
Ability to work remotely and perform outreach as needed
