As an RN Case Manager youll advocate for patients while collaborating closely with interdisciplinary health care partners facilities and families to ensure the best possible care experience from admission through discharge and beyond by connecting patients to resources that help them maintain and build on positive outcomes.

As a Case Manager you will also :

• Utilize clinical expertise discretion and independent judgment in assessing / reassessing facilitating care coordination utilization management and patient advocacy.
• Assure medical appropriateness criteria are met for status and level of care

Job Requirements

In addition to bringing your whole self to the workplace each day qualified candidates will need the following :

Physical Requirements -Medium Work - (Exert up to 50lbs force occasionally and / or up to 20lbs frequently and / or up to 10lbs constantly)
