In this role, you will be entrusted with the responsibility of evaluating patients in both inpatient and outpatient environments, tailoring unique treatment plans to meet their individual needs. You will administer interventions aimed at enhancing mobility, strength, balance, and overall function. Collaboration is key - you'll work closely with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes. Educating patients and their families on therapeutic goals, discharge planning, and home exercises will be part of your daily routine, along with documenting evaluations, treatments, and progress in line with facility guidelines. Your active participation in care team rounds and interdisciplinary conferences will ensure that every patient receives the coordinated care they deserve. To flourish in this role, a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program is required, along with state licensure as a Physical Therapist and preferably CPR certification. Specialized certifications in areas like neurological or geriatric care are beneficial, though not mandatory. Previous experience is strongly preferred as you will need to treat patients facing complex medical and mobility challenges, all while showcasing your exceptional communication and teamwork skills. Comfort with digital charting and clinical documentation systems is also essential.
