Redeemer Services, Inc is a small but ever-growing family-owned business in Woodbury, MN Washington County. We are a family-owned provider of services for the Department of Human Services, Ramsey, Hennepin County, and other counties across the Twin Cities metro area.

Originally called Redeemer Home Healthcare, Redeemer services, Inc. was founded in 2007 as a home health agency. Initially, we provided Relocation Coordination Services. In 2015, we began providing Moving Home Minnesota services. In 2016, we were approved to provide contracted case management for Ramsey County and in 2019, we began providing case management services for Hennepin County residents. Later in 2020, Redeemer began provider housing stabilization services.

Description

We are seeking a Housing Case Manager / Social Worker who will be responsible for assisting clients in finding long-term stable housing. The successful candidate will be responsible for determining prospective housing options based on the client's preferences and budget, removing any barriers that limit housing choices, and connecting clients with funding sources such as charity grants or government programs. The Housing Case Manager / Social Worker will also advocate for the client with prospective landlords, assist clients in applying for housing, and provide support once housing has been found.

Responsibilities :

• Determine prospective housing options based on the client's preferences and budget.
• Connect clients with funding sources such as charity grants or government programs.
• Remove any barriers that limit housing choices, such as eviction expungement.
• Advocate for the client with prospective landlords or current landlords.
• Assist clients in applying for housing.
• Provide support once housing has been found to ensure the client has long-term stable housing.
• Provide rides to your client to potential housing options.
