Case Worker

Busy New York Hospital is seeking a Case Worker to join their team. Responsibilities include : Conduct interviews and make initial assessments and referrals for social service recipients. Manage individual cases, including ongoing monitoring and documentation. Make home visits to assess clients’ needs and verify information. Provide health-related and social services to clients. Complete additional tasks as assigned by leadership. Qualifications : High School Diploma / GED required. At least 1 year of relevant experience. Strong communication, organizational, and problem-solving skills. Ability to work independently and handle sensitive client information. Please note that the salary range and / or hourly rate range of $30.00 - $40.00 is a good faith determination of potential base compensation offered to applicants at the time of this job advertisement and may be subject to modification in the future.
