A company is looking for a Clinical Pharmacist specializing in Medication Therapy Management, working remotely.

Key Responsibilities

Facilitates Medication Therapy Management (MTM) support and conducts comprehensive medication reviews

Analyzes medication profiles to optimize member medication regimens and ensure patient safety

Coordinates care for pharmacy-related issues and serves as a drug information resource

Required Qualifications

At least 3 years of experience in pharmacy, MTM, PBM, or managed care setting

Doctor of Pharmacy (PharmD) with an active and unrestricted license

Demonstrated knowledge in clinical pharmacology and disease management

Clinical skills related to Medication Therapy Management (MTM)

Ability to develop and maintain effective work relationships with various stakeholders
