BC / BE Internal Medicine, Family Medicine, or Geriatrician

Prisma Health, the largest not-for-profit healthcare provider in South Carolina, is seeking a BC / BE Internal Medicine, Family Medicine, or Geriatrician (preferred) to join our SeniorCare PACE (Program of All-Inclusive Care for the Elderly) team in Columbia, SC.

Prisma Health SeniorCare PACE provides comprehensive, patient-centered care to frail, elderly patients who qualify for nursing home-level care but prefer to remain in their communities. This interdisciplinary care model allows physicians to build meaningful relationships with patients, prevent hospitalizations, and improve quality of life for vulnerable older adults.

Position Highlights :

• Serve as the primary care provider for PACE participants, conducting regular assessments and managing their medical needs.
• Work within an interdisciplinary team of nurses, social workers, therapists, dietitians, and other specialists.
• Provide care in multiple settings (PACE center, participant’s home, hospitals, and skilled nursing facilities).
• Perform geriatric assessments addressing cognitive impairment, functional decline, polypharmacy, and end-of-life planning.
• Participate in care coordination and family discussions to align care goals with the participant’s preferences and needs.
• Collaborate with specialists and hospital teams when advanced interventions are needed.
• Teaching opportunities available with an academic appointment at the University of South Carolina School of Medicine, commensurate with experience.
• Knowledge of Medicare, Medicaid, and long-term care services is helpful.
