About the Role

As a Physician Assistant for Matrix, you will be responsible for conducting thorough assessments during in-home visits that last up to one hour. These assessments allow us to capture in-depth insights into our members' health, medical history, and social circumstances, ultimately improving their care quality. Your responsibilities will include :

• Performing adult and geriatric assessments encompassing medical history, diagnosis, treatment, health education, and necessary referrals, all documented per Matrix protocols.
• Administering and ordering point-of-care and lab-based tests as needed.
• Collaborating with physicians, case managers, social workers, and family members to coordinate care effectively.
• Engaging in patient education and follow-up in collaboration with Primary Care Physicians.
• Providing services in members' residences, including their homes or Skilled Nursing Facilities.
