Registered Nurse Case Manager Hospice

We are hiring for an RN Case Manager in Hospice.

This a Full Time position doing Home Visits for Hospice.

On-call rotation is currently for back-up only. This could change due to census or staffing. However, at this time, this position does not require an on-call rotation

As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.
