• Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate.\n
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.\n
• Note and carry out physician and nursing orders.\n
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.\nRequirements\n\nLicensure / Certification / Registration : \n
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.\n
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.\n\nEducation : \n
• Diploma from an accredited school / college of nursing OR Required professional licensure at time of hire required.\nAdditional Preferences\n
