Health Outreach Associate

A company is looking for a Health Outreach Associate with sales experience.

Key Responsibilities

Conduct initial screening consultations and assessments with patients and caregivers

Coordinate sessions with medical providers and home care aides

Enhance patient engagement and retention through compassionate outreach

Required Qualifications, Training, and Education

Experience in a healthcare environment, preferably with dementia or Alzheimer's patients

Sales experience with a goal-oriented mindset

Ability to work East Coast hours

Tech-savvy with the ability to troubleshoot and assist others with technology issues

Multilingual abilities, especially in Spanish, are a plus
