Overview

This position will provide outreach, health education, and contact tracing services with the goal of reducing infectious disease in the San Diego population. In partnership with the County of San Diego HHSA, Community Health Educators will provide as needed contact tracing services for infectious diseases to residents of SD County who are a confirmed/ probable case, close contact and/or a member of a hard-to-reach, vulnerable population. Community Health Educators will focus on underserved and under-represented communities.

Responsibilities

Community Outreach:

  • Identify and visit community sites, partner organizations, and events to engage with members of the public with the purpose of sharing public health information and providing linkages to available resources.
  • Support, community events to share relevant public health information, including preparing and transporting supplies for health education events.
  • Conduct health education activities around relevant public health topics in a group or one-on-one setting; for example, providing information about symptoms and prevention strategies at a health fair or workshop, modeling proper safety techniques (washing, masking), etc.
  • Provide connections to community resources to promote health, address healthcare barriers, receive translation/interpretation services, or address health-related social needs.
  • Identify service gaps and needs as defined by the community and communicate those to the team.

Health Education & Promotion:

  • Maintain up-to-date knowledge of relevant infectious diseases or other public health issues through mandatory training sessions, readings, meetings, and ongoing professional development.
  • Relay scientifically accurate health information to community members in ways that are culturally and linguistically appropriate.
  • Participate in required weekly team meetings, including sharing with the rest of the team key learnings from time in the field/community (i.e., emerging resource needs/ service gaps communities are experiencing, common questions/concerns, etc.)
  • Support the development of accurate, visually engaging, and language appropriate health education materials in conjunction with other project staff.

Contact Tracing:

  • Conduct contact interviews (primarily via phone) and provide notification of exposure to someone that the County has verified as a confirmed case.
  • Conduct follow-up and monitoring activities (according to protocol).
  • Make referrals to other resources, as needed or appropriate.

Data Collection and Entry

  • Document all information collected during interviews completely and according to established protocols.
  • Data entry into a project database
  • Ensure HIPAA compliance for all project related data.

POSITION SPECIAL RESPONSIBILITIES:

  • Must be able to independently travel locally around San Diego County
  • Ability to communicate effectively, both orally and in writing
  • Detail-oriented with excellent organizational skills
  • Ability to follow established protocols.
  • Ability to work both individually and as part of a team.
  • Ability to effectively and respectfully engage with diverse groups of people.
  • Responsible for accurately entering any data collected into a secure database within expected timelines and according to protocol.
  • Promote a culture of excellence, inclusion, learning, support, diversity, and innovation.
  • Must be available to work occasional evenings and weekends.

Qualifications

  • Four years of related work experience or an undergraduate degree in a related field
  • Arabic, Chinese (Mandarin), Korean, Persian (Farsi/Dari), Somali, Spanish, Tagalog, and/or Vietnamese are especially desired.
  • Experience working in San Diego County and community settings (community outreach, family/public support services, Advocacy and/or related services) a plus.
  • Ability to demonstrate a high level of cultural humility and respect.
  • Strong interest in public/community health
  • Access to reliable transportation
  • Access to a quiet and private workspace when working remotely.
  • A passion for the mission and values of Global Communities
  • Must have U.S. work authorization.
