Location : Various locations across  California As a Home Health Nurse, you will provide personalized nursing care topatients in their homes, promoting recovery and enhancing their quality oflife. Your role will involve assessing patients’ needs, administeringtreatments, and supporting families in a comfortable and familiar environment. Key Responsibilities :

• Conduct comprehensive assessments of patients’health status, including physical exams, medical history reviews, and homeenvironment evaluations.
• Develop and implement individualized care plans incollaboration with healthcare teams, tailoring interventions to meet eachpatient's unique needs.
• Administer medications, wound care, and othertreatments as prescribed, ensuring adherence to safety and infection controlprotocols.
• Monitor patients’ progress and responses totreatment, making adjustments to care plans as necessary and communicatingfindings to healthcare providers.
• Educate patients and their families aboutmanaging medical conditions, medications, and necessary lifestyle changes,providing resources and support.
• Maintain accurate and up-to-date patientrecords, documenting assessments, interventions, and patient outcomes.
• Coordinate care with other healthcareprofessionals, including physical therapists, social workers, and physicians,to ensure comprehensive support.
• Stay informed about best practices in homehealthcare and changes in regulations to provide high-quality care.
