Description :

Physical Therapist for Home Health.

Position involves providing therapy services to patients in their homes.

Approximately six visits per day are expected.

Patient types include those with chronic diseases, requiring wound care, IV therapy, and disease management.

Skills needed include assessment, disease teaching, and experience with OASIS documentation.

Previous experience with geriatric and adult populations is preferred.

Transportation is necessary, along with a valid driver's license and proof of insurance.

Technology used includes Samsung Note 9 phones and an EMR system.

Support will be provided by departmental leadership.

No weekend rotation or on-call duties are required unless requested.
