Territory : Targeted and manageable area of Oakland. We also have ongoing needs in San Mateo, Santa Clara, Contra Costa and Alameda Counties.

21st Century HHS is expanding rapidly throughout the East Bay. We anticipate offering Clinicians and Therapist targeted and manageable territories no later than early 2023, in all areas of Contra Costa and Alameda Counties. However, you don't have to wait until 2023 to join us! We are offering immediate opportunities for individuals to train in Oakland and transition to preferred and convenient territories once available.

What you'll do as a 21st Century Home Health Physical Therapist :

Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment. You will :

• Examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability.
• Work with patients to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

Wage Range $70 - $90 Pay Per Visit Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers comprehensive health benefits and 401k (Up to 4% match)
