Position Summary :

We are seeking a dedicated and experienced Nurse Practitioner (NP) to join our healthcare team in Victorville, CA. The ideal candidate will have strong expertise in wound care and experience in managing patients across multiple specialties. This role requires the NP to work onsite and collaborate with physicians, clinicians, and care teams to provide high-quality, patient-centred care.

Key Responsibilities :

• Provide comprehensive assessments, diagnoses, and treatment plans for patients with acute and chronic wounds.
• Perform wound debridement, dressing changes, and advanced wound management techniques.
• Conduct patient home visits as required to deliver wound care and multispecialty support .
• Collaborate with multidisciplinary teams to develop individualized care plans.
• Manage patients across multiple specialties (primary care, internal medicine, geriatrics, etc.).
• Educate patients and caregivers on wound prevention, treatment, and overall health management.
• Prescribe medications, order diagnostic tests, and interpret results.
• Document all patient encounters and maintain accurate medical records.
• Ensure compliance with clinical guidelines, safety protocols, and regulatory standards.
