Job Title : Registered Nurse, Home Care Department : ElderOne Location : Monroe County Hours Per Week : 20 hours Schedule : Eve / Night SUMMARY : As a Registered Nurse in Home Care, your exceptional ability to teach, counsel and empathize with each patient leads to an enhanced quality of life. RESPONSIBILITIES :

• Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;
• Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
• Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff REQUIRED QUALIFICATIONS :
• Nursing Diploma required
• License and current registration to practice as Registered Professional Nurse in New York State
• Current NYS Driver's license PREFERRED QUALIFICATIONS :
• 6 months- 1 year experience conducting UAS-NY Community Assessment preferred.
• Bachelors degree of Science in Nursing preferred.
• 1 year of experience working with a frail or elderly population preferred.

PAY RANGE : $73,528.00 - $98,072.00 CITY : Rochester POSTAL CODE : 14617 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
