A company is looking for an Out of Network (OON) Clinical Case Manager (CM) - Remote (CA Only) - Per diem - Variable.\n\nKey Responsibilities\n\nCoordinate quality healthcare services in out of network facilities and repatriate patients into the healthcare system\n\nAssess, develop, implement, and monitor comprehensive care plans in collaboration with interdisciplinary teams\n\nFacilitate proactive discharge planning and ensure timely communication among healthcare team members\n\nRequired Qualifications\n\n3 Years Clinical RN Experience in the ICU, Trauma, or Emergency Room setting\n\n3 Years RN Acute Care, Ambulatory Care, or comparable Case Management experience\n\nCalifornia Registered Nurse (RN) license\n\nDriver's License - CA Department of Motor Vehicles
