Schedule : Day Shift 4x10-Hour (05 : 00 AM 03 : 30 PM) Variable start times (5AM 6AM 10 : 30AM or 3 : 30PM); weekends required

Job Description

Summary :

The Inpatient Pharmacist is responsible for providing comprehensive pharmaceutical care to hospitalized patients. This includes verifying physician orders preparing and dispensing medications (including IV and chemotherapy) providing clinical dosing support and ensuring compliance with hospital policies and regulatory standards. The pharmacist works closely with the interdisciplinary healthcare team to optimize patient outcomes and promote medication safety within the inpatient setting.

Responsibilities :

Review and verify medication orders for appropriateness dosing accuracy and potential drug interactions.

Prepare compound and dispense medications including IV admixtures chemotherapy and hazardous drugs per USP and facility protocols.

Provide clinical consultation on medication therapy dosing adjustments and monitoring.

Manage therapeutic drug monitoring programs (e.g. Warfarin Vancomycin Aminoglycosides).

Oversee controlled substance handling and ensure compliance with regulatory standards.

Collaborate with nursing and medical teams to ensure timely and accurate medication administration.

Educate clinical staff on pharmacology new medications and safe medication practices.

Participate in interdisciplinary rounds and assist with formulary and quality improvement initiatives.

Key Requirements :

Minimum 1 year of recent inpatient hospital pharmacy experience required.

Must be proficient in Epic EMR.

IV and sterile compounding experience required.

Chemo / hazardous drug handling preferred.

Must be available for rotating weekends and flexible start times.

Must be willing to float between like units.

Pending license not accepted.

RTO must be declared at submission.

Qualifications

Education :

Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy (RPh) from an accredited program.

Experience :

At least 1 year of inpatient acute care pharmacy experience required.

Trauma Level II hospital experience preferred.

Travel experience not required open to strong local candidates.

License :

Active and unrestricted State Pharmacist License required at submission.

IV Certification required.

Chemo / Hazardous Drug Certification preferred.

Skills :

Intravenous preparation and sterile compounding

Antibiotics and Anticoagulant dosing

Chemotherapy preparation

IV piggyback and push medication administration

Packaging of antineoplastic agents

Clinical dosing (cardiac geriatric pediatric renal)

Pharmacokinetic dosing and therapeutic drug monitoring

Familiarity with Pyxis or similar medication dispensing systems
