The Referrals Coordinator provides care coordination for patients and families by responding to calls for referrals. Participates in quality improvement activities within the practice.

Primary Responsibilities :

• Daily, check the referrals from the output queue from the previous day of business Maintain the queue in a timely manner
• Returns patient calls with referral information promptly
• Documents referral information in the patient chart under additional results / procedure results
• Obtain authorizations as needed / required
• Promptly schedules all “urgent / same day” appointments
• Answer incoming calls and handles according to protocol
• Check voicemail, return and handle those calls
• Attends and participates in team meetings.
• Uses open and structured communication with providers, staff and patients.
• Trained in scanning and indexing
• Work reports
• Other duties as assigned.
