Job Description
The Referrals Coordinator provides care coordination for patients and families by responding to calls for referrals. Participates in quality improvement activities within the practice.
Primary Responsibilities :
- Daily, check the referrals from the output queue from the previous day of business Maintain the queue in a timely manner
- Returns patient calls with referral information promptly
- Documents referral information in the patient chart under additional results / procedure results
- Obtain authorizations as needed / required
- Promptly schedules all “urgent / same day” appointments
- Answer incoming calls and handles according to protocol
- Check voicemail, return and handle those calls
- Attends and participates in team meetings.
- Uses open and structured communication with providers, staff and patients.
- Trained in scanning and indexing
- Work reports
- Other duties as assigned.
Desired Education and Experience :
High School Diploma or G.E.D.Experience in medical office referrals preferred.Previous experience working with electronic medical records preferred.Knowledge, Skills and Abilities :
Knowledge of reporting systems, HIPAA requirements.High level of professionalism, excellent communications skills.Demonstrated knowledge and skills with various (medical) software, PC, and general office equipment use.Physical Requirements and Work Conditions :
Sitting for extended periods of time at a desk with a computer.Operating a personal computer is a routine part of the job.Good vision for utilizing computer.Work environment is fast paced and may be stressful at times.
