The Colorado Rural Health Center (CRHC) is seeking a Program Coordinator with the passion and skills to support the Rural Health Transformation Program (RHTP) implementation and day-to-day operations coordinating program activities facilitating communication with applicants, partners, and internal team members and closely working with the RHTP Program Manager to deliver high quality technical assistance for all programs and activities within the RHTP.

CRHC is a non-profit organization that serves as the State Office of Rural Health and has been serving the state of Colorado since 1991. CRHC's mission is to enhance healthcare services in Colorado by providing information, education, linkages, tools and energy toward addressing rural healthcare issues. As an RHTP Program Coordinator, you will:

• Support grants management activities by assisting with tracking, documentation, and reporting of RHTP contract deliverables to help ensure compliance with RHTP requirements and the expectations of CRHC's partner, the Colorado Department of Health Care Policy and Financing (HCPF).
• Assist with the implementation and ongoing coordination of RHTP technical assistance activities in alignment with contract guidelines.
• Collaborate with the CRHC Outreach Department to support development and distribution of brochures, fact sheets, toolkits, marketing flyers, educational handouts, and social media content. Assist with communicating available resources, trainings, peer-to-peer learning opportunities, and CMS and HCPF updates to RHTP applicants and grantees.
• Coordinate logistics and communications with internal team members, HCPF, applicants, grantees, and external partners for meetings, webinars, and site visits as needed.
• Support planning and execution of workshops, conferences, site visits, and other program-related events statewide and in the Denver area; attend program-related meetings and workshops, in person or remotely; and assist with preparation, materials, and follow-up.
