Position Summary
Assists clients by addressing social and financial health care needs, provides general supervision of clients, makes appropriate medical placement(s), and conducts health care needs assessments.
Pay Grade : 45
Salary Commensurate with Experience and Education
Minimum Qualifications
100% -
1.Three (3) years of experience in a social services environment, statistical work, or related field; AND
2. High school diploma or possess GED, OR
3. Two (2) years of experience in a social services environment, statistical work; or related field; and one (1) year of college study (30 semester hours or 36 quarters) from an accredited college or university.
4. PROOF OF EDUCATION, TRAINING, AND / OR EXPERIENCE IS REQUIRED.
Duties and Responsibilities
1. Confers with caregivers and creditors and writes / signs appropriate correspondence.
2. Investigates and negotiates clients’ bills.
3. Monitors personal, real property and completes property inventory.
4. Maintains medication inventory.
5. Qualifies client(s) for Medicaid spend down.
6. Maintains and updates departmental reports.
7. Attends care conferences.
8. Conducts quarterly assessments.
9. Recruits, trains, and supervises volunteers for the Public Conservator Program.
10. Participates in regular Coordinated Community Response meetings.
11. Performs other related duties as required or directed.
KSA's
KSAs -
1. Knowledge of administrative functions, programs, policies and procedures.
2. Knowledge and skilled in the use of Microsoft Office Suites (Word, Excel, Outlook).
3. Knowledge of organizational, financial (including budgeting, accounting and / or auditing), purchasing and / or personnel management techniques.
4. Knowledge of Shelby County program and policies and services and related federal, state, and local programs.
5. Ability to plan, organize and maintain an even flow of work.
6 Ability to provide effective customer services in a fast-paced environment.
Special Requirements
Must pass background check to include : TBI / FBI Criminal check, Tennessee Abuse Registry, Tennessee Felony Offender Registry, Adult Offender Registry and the National and Tennessee Sexual Offender Registry.
