A company is looking for a Care Manager II.
Key Responsibilities
Assess members' care coordination and management needs through comprehensive evaluations
Develop and implement individualized care plans, coordinating with various health services
Document progress and facilitate communication among members, caregivers, and healthcare providers
Required Qualifications, Training, and Education
Bachelor's Degree in Nursing is preferred; active RN license in Michigan is required
2 to 3 years of Labor and Delivery clinical experience required
3 to 5 years of Case Management experience preferred
Certified Case Manager (CCM) is preferred but not required
Current and unrestricted Registered Nurse (RN) licensure in Michigan is required
