Key Responsibilities

Make outbound calls to patients scheduled for home or telehealth visits

Initiate chart preparation to support providers

Enter patient data into EMR systems and conduct phone-based patient assessments

Required Qualifications

Active LVN / LPN license

High School diploma or GED
