Descripción del trabajoQualifications : Active Licensed Vocational Nurse (LVN) license in the state of practice. Previous experience in hospice or palliative care is preferred. Strong communication and interpersonal skills. Compassionate and empathetic approach to patient care. Ability to work effectively in a team environment. Basic computer skills. Responsibilities : Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients. Administer medications and treatments as prescribed by the physician. Monitor and assess patient's condition and report changes to the healthcare team. Provide emotional support and education to patients and their families. Ensure all documentation is completed accurately and in a timely manner. Maintain patient confidentiality and privacy at all times. Follow all regulatory standards and company policies.
