Descripción del trabajoLicensed M.D. or D.O. in the state of Colorado Controlled substance license with current DEA license number Eligible or currently possess a Medicare / Medicaid NPI number. Board certified in primary care specialty such as Internal Medicine or Family Medicine. Preferred additional certification in Geriatrics, Hospice & Palliative Medicine, HPM Medical Director certification, or other relevant senior care certification. 5 years’ clinical experience required. 5 years of supervisory and administrative experience preferred. Valid Colorado driver’s license, unlimited access to reliable transportation, and adequate automobile insurance maintained throughout employment. Excellent interpersonal and communication skills. Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions. Current CPR certification is maintained throughout employment. Adequate computer skills, experience in EMRs, and Microsoft office products Effectively work with and build relationships with others. Personal commitment to collaborating with diverse customers and colleagues. Competitive salary range of $212,347-$234,915 annually. Full-Time, Monday-Friday, 8 : 00am-5 : 00pm schedule. Cigna Health, Dental, and Vision insurance. Immediately begin accruing PTO and medical leave. PACE Associate Medical Director (MD or DO)

What is PACE?
PACE ( P rogram of A ll-Inclusive C are for the E lderly) was created to answer the many problems around caring for frail, and medically complex seniors. The PACE model is intended to keep individuals living safely and comfortably in their homes whenever possible.
HopeWest PACE participants receive wrap around services that include but are not limited to in-home care services, transportation to a HopeWest PACE center for primary medical care, social and recreational activities.

The Unique Responsibilities of the Associate Medical Director at HopeWest PACE :
- Serves as the clinical leader and resource for complex cases to internal and external clinicians.
- Provides direct, expert medical care to participants in a variety of settings such as clinic, day center urgent care, home, rehab, nursing home, and assisted living facilities.
- Actively collaborates with the entire PACE interdisciplinary team for the highest quality care of each participant.
- Assists with developing, planning, and providing in-service education for all PACE care team.
- Assists with the development of policies and procedures, establishing standards of care, and participating in quality assurance, compliance program, emergency procedures, as assigned and in conjunction with the PACE Medical Director.
- Participates in quality monitoring, quality improvement, and process improvement projects.
