LICENSED SOCIAL WORKER

The Social Worker is responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being. Education Bachelor's degree in Social Work or similar professional qualifications; Current Social Work license as regulated by state law. Position Requirements Two years of social work supervised experience working directly with geriatric clients in a health care setting, desirable.

Job Responsiblities

Develops preliminary and comprehensive assessments, written plans of care, discharge plans and progress notes and reviews and revises these plans as necessary

Provides resident, family and / or resident representative with information concerning resident’s rights and advance directives

Develops an effective working relationship with the resident, family, team members, physicians, community and others to collaborate, advocate and problem solve on behalf of the resident to allow him / her to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being

Ensures residents have access to prescriptions, durable medical equipment (DME), and other services as identified upon discharge

Refers residents to education regarding the healthcare delivery and reimbursement systems, prescription drug programs, health & wellness programs, community agencies, public and private organizations, housing options, and other services, as appropriate

Participates in committees and Performance Improvement Projects

Possesses excellent analytical and team building skills, as well as the ability to prioritize and work independently

Demonstrates awareness of medical / legal issues, patient rights and compliance with standards of regulatory and accrediting agencies

Full-time position

Previous Skilled Nursing Experience REQUIRED
