Internal Medicine physician job in New York : Utica, New York, Post-Acute Care Physician Opportunity Clinical Role :

• Thisfull-time primary carephysicianrole will include patient care rounding in apost-acute care settingin askilled nursing facility (SNF)located in theUtica, NY, area. You will round 5 days a week with the flexibility to choose your rounding schedule.
• Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
• Collaborate with clinicians, facility leaders and others regarding care and services
• Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
• Provide oversight, guidance, and mentorship to our nurse practitioners
• Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
• Minimum of 1-year primary care experience
• Post-acute care experience a plus, but not require
• Hospitalists, internal medicine, family medicine, emergency medicine and related specialties are encouraged to apply
• Active and unrestricted New York license or application in process
• Active and unrestricted DEA / prescriptive authority or application in process
• Expected compensation is $183,222 to $221,114 annually with no cap on productivity income potential.
